INFORMED CONSENT DECLARATION

I, the undersigned, as a participant in an electric motorbike tour organized by "Extreme Cross" Ltd., UIC 207559485, hereby declare:

1. I have received a full briefing on operating the SUR-RON electric motorbike and the safety rules.
2. I participate voluntarily and of my own free will, being aware that riding a motorbike in mountain and natural terrain carries risks, including falls, injuries, material damage or more serious incidents.
3. I confirm that:
   • I am in good physical and mental condition
   • I have no medical contraindications
   • I am not under the influence of alcohol, narcotics or medication that affects reactions
4. I received the motorbike and equipment in technically sound condition. Bike No.: ___
5. I undertake to:
   • follow the tour guide's instructions
   • ride in accordance with the terrain and conditions
   • wear protective gear at all times
6. It is prohibited to:
   • perform jumps, stunts and dangerous manoeuvres
   • ride outside the designated route
   • enter restricted zones
   • hand over the motorbike to other persons
7. I agree that the tour guide may terminate my participation if I fail to follow the rules or create a risk to myself or other participants.
8. In the event of an incident caused by non-compliance with instructions, I undertake to cover the costs of repair, transport or other resulting damages.
9. I accept the risks associated with the natural environment, terrain and weather conditions.
10. I release "Extreme Cross" Ltd., its employees and partners from liability for incidents during the tour, except in cases of intentional actions or gross negligence.
11. I have been informed that no Accident insurance is provided.
12. I GIVE / DO NOT GIVE consent for photos and videos from the tour to be used for marketing purposes.
13. I agree that the company may process my personal data in accordance with GDPR.

The renter bears full responsibility for damages caused by:

• improper use
• negligence
• failure to follow the guide's instructions
• riding outside the permitted route

In case of total loss or theft: €4,950.

For partial damages, the cost of the part + labour is owed:
