INFORMED CONSENT DECLARATION

This declaration is completed and signed on paper on site, before departure — each participant signs their own. For participants under 18 it is signed by a parent or guardian. The checkbox in the online booking form is acceptance of the Terms and Conditions and an acknowledgement that you know about the declaration; it does not replace it. A participant without a signed declaration does not start the tour.

I, the undersigned, as a participant in an electric motorbike tour organized by "Extreme Cross" EOOD, UIC 207559485, hereby declare:

1. I have received a full briefing on operating the SUR-RON electric motorbike and the safety rules.
2. I have been informed that the tour is always conducted with an instructor-guide, whose instructions are binding on me at all times.
3. I participate voluntarily and of my own free will, being aware that riding a motorbike in mountain and natural terrain carries risks, including falls, injuries, material damage or more serious incidents.
4. I confirm that:
   • I am in good physical and mental condition
   • I have no medical contraindications
   • I am not under the influence of alcohol, narcotics or medication that affects reactions
   • the details I have provided in this declaration are true
5. I received the motorbike and equipment in technically sound condition, including protective gear in a suitable size. Bike No.: ___
6. I undertake to:
   • follow the tour guide's instructions
   • stay with the group, without overtaking the guide or leaving the group
   • ride in accordance with the terrain and conditions
   • wear protective gear at all times
7. It is prohibited to:
   • perform jumps, stunts and dangerous manoeuvres
   • ride outside the designated route or enter restricted zones
   • carry a passenger on the motorbike
   • consume alcohol or intoxicating substances during the tour
   • hand over the motorbike to other persons
8. I undertake to notify the guide immediately in the event of an incident, a fall, damage to the motorbike or a deterioration of my physical condition.
9. I agree that, if necessary, first aid may be administered to me and that the emergency services and/or the Mountain Rescue Service may be alerted.
10. I agree that the tour guide may terminate my participation without a refund of the amount paid if I fail to follow the rules or create a risk to myself or other participants.
11. In the event of an incident caused by non-compliance with instructions, I undertake to cover the costs of repair, transport or other resulting damages.
12. I am liable for damages I cause to third parties and their property.
13. I accept the risks associated with the natural environment, terrain and weather conditions.
14. I release "Extreme Cross" EOOD, its employees and partners from liability for incidents during the tour, except in cases of intentional actions or gross negligence.
15. I have been informed that no Accident insurance is provided (see the "IMPORTANT" box above).
16. □ I GIVE □ I DO NOT GIVE (for minors — ticked by the parent/guardian) consent for photos and videos featuring me to be used by "Extreme Cross" EOOD for advertising on the website and social media. I may withdraw my consent at any time at info@vratsaadventures.bg or +359 889 491 639; withdrawal takes effect for the future. A refusal does not affect my participation in the tour or its price.
17. I agree that "Extreme Cross" EOOD may process my personal data in accordance with Regulation (EU) 2016/679 (GDPR) and the Bulgarian Personal Data Protection Act — for the purpose of organising the tour and for a period of up to 3 years.

The renter bears full responsibility for damages caused by:

• improper use
• negligence
• failure to follow the guide's instructions
• riding outside the permitted route

In case of total loss or theft: €4,950.

For partial damages, the cost of the part + labour is owed:

All other parts — at the importer's catalogue price. Lost or damaged protective gear (helmet, protectors) — at catalogue price.

In case of dispute, the value of the damage is determined by an authorised service centre. Amounts due are payable within 7 days of the damage being established.

The renter must not leave the motorbike unattended during stops along the route.

Bulgarian law applies to all matters not covered herein. Disputes are settled by agreement, or failing that, by the competent court.
